Air Accident Investigation

airaccident.jpgProsolve Ltd provides comprehensive Air Accident Investigation services and is experienced in the investigation of both aeroplane and helicopter air accidents and incidents.

In the past, Prosolve has undertaken many of these investigations in accordance with procedures set out by the International Civil Aviation Organisation (ICAO) which emphasises accident prevention as             opposed to blame.

Services & Capabilities


Prosolve Ltd is able to investigate both the operational (flying) and engineering (forensic) aspects of an air accident and if necessary engages specialist individuals and consultants to assist as required. 

An example of such a specialist is Paul Waterhouse of Central Aero Maintenance, Hamilton, New Zealand. Paul is a Licensed Aircraft Maintenance engineer(LAME) and has maintenance ratings in numerous classes of fixed and rotary wing aircraft.

Another example is Les Boulton of Les Boulton and Associates. Les is an experienced metallurgist with a specialty in corrosion prevention and assessment. Comprehensively determining a causal profile often requires open brainstorming and reconstruction meetings with associates such as these.

Prosolve Ltd is able to identify and assess these difficult factors in a manner that properly represents the sensitive interests of those involved in the accident, yet captures the opportunity that the accident participants have provided by researching and summarising lessons needed to prevent future recurrances. 

We recognise the work of notable authors such as James Reason and Sydney Dekker and have found these works invaluable in terms of understanding the nature and effect of intangible human factors and the complex mechanisms of organizational accidents.
